Adult Care and Support Functions

The Assessment & Eligibility Criteria procedures should be used when a young person in transition is:

1. 18; or
2. Nearly 18; and
3. Is likely to have needs for Adult Care and Support.

They should also be used when a young carer is 18 (or nearly 18) and likely to have needs for support.
